Due to internet email harvesting I've had to remove the Order Form. 

To place an order please just send an email:     fantasyphoto@prtcnet.com

Please include the following information:

 - Your name and email address

 - The image number and date

 - The product that you wish to order (i.e., tshirt, keychain, etc).  Include quantity.

 - Any special graphics, text and sizes

Once your design has been created you will receive a preview by email for approval. 

The email will also include your total cost, to include shipping. 

There is no charge for the artwork, only for the products that you purchase. 

You will not be charged until you approve the design.
